Buprenorphine Taper — Week 4

Week 4 Journal

Week 4 has been a continuation of structured tracking, symptom awareness, and controlled medication use. The focus has remained on maintaining stability while managing pain, fatigue, and emotional fluctuations.

There has been a clear pattern of fluctuating pain, particularly in the lower back and sciatic region. Pain increases with activity, sitting, and stress, and settles with movement, rest, and medication. Fatigue has been a consistent feature, especially in the afternoon, and at times has required rest.

Emotionally, there have been periods of stress and frustration, particularly during travel and physical strain, but these have resolved without destabilising the overall week.

Day 1

Medications: Paracetamol 1000 mg; Buprenorphine 800 mcg (split across the day); Diazepam 20 mg (split doses).

Reflection: A physically and emotionally demanding day. Pain fluctuated with activity and sitting. Travel and stress increased symptoms temporarily, but medication and rest helped maintain control. The day remained stable overall.

Day 2

Medications: Paracetamol 1000 mg; Buprenorphine 600–800 mcg (split dosing).

Reflection: Pain present but manageable. Energy dipped in the afternoon, consistent with previous patterns. Emotional stress briefly increased symptoms but settled without issue.

Day 3

Medications: Paracetamol 1000 mg; Buprenorphine ~600 mcg; Diazepam 10 mg.

Reflection: Muscle spasm and lower back pain more noticeable. Diazepam helped reduce tension. Pain improved with rest and heat. Mood remained stable.

Day 4

Medications: Paracetamol 1000 mg; Buprenorphine ~600 mcg.

Reflection: Relatively stable day. Pain present but controlled. Fatigue noticeable in the afternoon. No destabilisation during taper.

Day 5

Medications: Paracetamol 1000 mg; Buprenorphine ~600 mcg.

Reflection: Slight increase in awareness of pain, consistent with taper. No major escalation. Energy remained low but manageable.

Day 6

Medications: Paracetamol 1000 mg; Buprenorphine ~600 mcg; Diazepam 10 mg (as required).

Reflection: Pain increased with activity but settled with rest. Anxiety remained manageable. The body continues to respond predictably to load and recovery.

Day 7

Medications: Paracetamol 1000 mg; Buprenorphine ~600 mcg.

Reflection: Stable and reflective day. Pain present but manageable. Energy low but consistent. Clear progress in structure, awareness, and control.

Closing Reflection

Week 4 demonstrates ongoing stability within fluctuation. Pain, fatigue, and anxiety are present but manageable. Medication has been used in a controlled and structured way, supporting function without loss of control.

There is clear evidence of adaptation and resilience. The taper is progressing with awareness and without destabilisation.
